Fundraiser Form

Thanks for raising funds for your cause through a Lion’s Choice dine-in fundraiser. We are pleased to partner with many not-for-profit organizations by simply gathering their supporters to dine with us on a designated date and time, during which 20% of your net sales will be donated to the group. 

We need at least four weeks’ notice to schedule your event and for you to effectively promote your fundraiser. Our team will respond to your request within 10 business days. After your fundraiser, please allow at least 14 days for your proceeds to be mailed to you.

  • I understand that for the purchases to count towards my fundraiser, my supporters must either 1) verbally tell our cashier of their participation, 2) show a physical fundraising flyer, or 3) show a digital flyer on a device. Twenty percent of the total net sales from my event will be donated. No soliciting is allowed inside or outside of Lion’s Choice. My fundraising date is not confirmed until I receive an official confirmation email from Lions Choice.
